STAR Medicaid & CHIP ID Cards

You may not think about your health insurance ID card very often. After all, it probably spends most of its time in your wallet - until you, your doctor, or another medical provider really need it.

Your member ID card (or health plan ID card) is proof that you have health insurance. Healthcare providers use the information from your member ID card to confirm they are part of your plan’s network and to bill your health plan for your care.

You or your child will get a Parkland HEALTHfirst ID card from us when you join Parkland HEALTHfirst. Each of your children who are on Medicaid will have a different card. You will not get a new Parkland HEALTHfirst ID card every month. We will also send you a new ID card if you change your primary care provider or if you move.

STAR ID Card Example

When you or your child is enrolled with Parkland KIDSfirst, you or your child will get an ID card from us. You or your child will not get a new ID card every month. If you call us to change your/your child’s primary care provider or if your/your child’s copay changes, you or your child will get a new ID card.

CHIP ID Card Example

You will get a Parkland CHIP Perinate Newborn ID card from us when your baby joins Parkland CHIP Perinate Newborn. The Member ID is the same as your child's CHIP ID number. If you have more than one baby, each baby will have a separate ID card.

CHIP Perinate ID Card Example

The front of the ID card shows important information about you or your child. It also has the name and phone number of your/your child's primary care provider.

The back of the card has more information and important phone numbers for you to call if you need help.

The Parkland KIDSfirst ID card will have co-payment information, if applicable. The Parkland CHIP Perinate Newborn ID card will show no co-payment information.

Always carry your/your child’s ID card with you when going to see the doctor. You will need it to get healthcare for you or your child. You must show it each time you or your child gets services.

If your HEALTHfirst ID Card gets lost, please call Member Services toll-free right away at 1-888-672-2277, TTY 1-800-735-2898, to get a new ID card mailed to you.

If your child’s Parkland KIDSfirst or Parkland CHIP Perinate Newborn ID Card gets lost, please call Member Services toll-free right away at 1-888-814-2352, TTY 1-800-735-2989, to get a new ID card mailed to you.

If you need to replace your HEALTHfirst ID card, please call Member Services toll-free right away at 1-888-672-2277, TTY 1-800-735-2898, to get a new ID card mailed to you.

If your child’s Parkland KIDSfirst or Parkland CHIP Perinate Newborn ID card gets lost, please call Member Services toll-free right away at 1-888-814-2352, TTY 1-800-735-2989, to get a new ID card mailed to you.
